Job Overview
Seeking a Registered Nurse (RN) to provide nursing care in a rehabilitation setting. The RN will support patients throughout their rehabilitation stay by providing safe, compassionate, and patient-centered care, monitoring patient conditions, administering medications and treatments as ordered, and collaborating with the interdisciplinary healthcare team to support recovery and functional goals.
Roles & Responsibilities
- Provide comprehensive nursing care to rehabilitation patients.
- Assess patients' conditions and monitor changes in health status.
- Develop, implement, and evaluate individualized nursing care plans.
- Administer medications and treatments according to physician orders and facility policies.
- Monitor vital signs and patient responses to treatment.
- Assist patients with activities of daily living and rehabilitation goals as appropriate.
- Provide patient and family education regarding medications, treatment plans, and ongoing care.
- Monitor for changes in patient condition and communicate concerns promptly to the appropriate healthcare provider.
- Collaborate with physicians, therapists, nursing staff, and other members of the interdisciplinary team.
- Maintain accurate and timely patient documentation.
- Follow infection prevention, safety, and quality standards.
- Support safe patient mobility and fall-prevention measures.
- Assist with discharge planning and transition-of-care needs as appropriate.
- Provide emotional support and encouragement to patients and families throughout the rehabilitation process.
- Follow facility policies, procedures, and applicable nursing standards.
Requirements
- Active and unrestricted Registered Nurse (RN) license
- Rehabilitation nursing experience preferred
