Job Summary (List Format):



- Evaluate and treat patients of all ages, modifying care plans as needed.

- Serve as a role model and clinical resource for staff and students.

- Perform patient transfers safely, using proper body mechanics to prevent injury.

- Demonstrate empathy, respect, and engagement with patients and colleagues.

- Design and adjust individualized care plans, integrating goals, treatments, and discharge planning.

- Educate patients, families, caregivers, students, and healthcare providers using effective teaching methods.

- Conduct thorough patient examinations and determine physical therapy diagnoses and outcomes.

- Document care and patient progress according to clinical and organizational policies.

- Screen patients to assess need and effectiveness of therapy services.

- Utilize and supervise support personnel in compliance with legal and ethical standards.

- Provide feedback and monitor the care delivered by support personnel.

- Implement intervention plans using functional activities tailored to patient needs.

- Modify therapeutic approach based on patient behavior and performance.

- Use standardized outcome measures to assess functional performance.

- Collaborate with patients, families, and care teams throughout the rehabilitation process.

- Supervise and provide guidance to Physical Therapist Assistants (PTAs) and rehabilitation technicians.

- Clearly communicate patient status, activity recommendations, and discharge needs to the medical team.



Requirements:

- Bachelor's Degree in Physical Therapy.

- Illinois Physical Therapist (PT) License.

- BLS Certification (American Heart Association).

- New graduates welcome.

- English proficiency (speak, read, write).



Schedule:

- Monday to Friday, 8:30am – 5:00pm.

- One weekend per month, no holidays.

- 13-week contract.
