General Role and Function:

The LVN is responsible for the overall care and medical supervision of clients while attending the Alzheimer’s Services center. Coordination of medication management and administration with family members, responsible parties, and treating physicians. This position may be assigned to any Alzheimer’s Services center and work schedule may change as needed.

Key Tasks and Responsibilities:

  • Assess the clients’ nursing and medical needs by working with the family members, responsible parties, and treating physicians.
  • Develop and maintain clients’ individual care plan.
  • Maintains manager and employee confidence by keeping human resources information confidential.
  • Obtain physician’s orders for medication and treatments to be administered to clients while at the center.
  • Determine whether self-administered medications have been appropriately taken, applied, or used as directed by the clients’ treating physician.
  • Enter daily or monthly progress notes on medical care provided.
  • Administer medication and treatment as authorized by treating physician.
  • Collaborate with the Director to ensure the activity schedule is implemented. Conduct activities with individual clients and groups as needed.
  • Ensure that quarterly caregiver surveys are completed for reporting purposes.
  • Provide oversight to the Activity Assistants/CNA’s (Certified Nursing Assistants) as needed.
  • Provide monthly in-service training to direct service staff as required by the Texas Department of Health and Human Services.
  • Maintain medical records of clients and update them monthly.
  • Provide health education to ensure the center staff are aware of Alzheimer’s disease and other diseases pertaining to seniors.
  • Document all incidents and ensure that all incident reports are maintained on file and reported to the director.
  • Assist with monitoring compliance with licensing requirements and regulations for Day Activity and Health Services facilities.
  • Lead implementation efforts for infection prevention and control.
  • Assist in the development revision of emergency preparedness and response policies and procedures.
  • Adhere to the policies and procedures of the center and organization.
  • Assist with the orientation, training, and supervision of the center volunteers.  
  • Other duties as assigned by management. 

Qualifications and Education Requirements:

  • Must have an active LVN license with the Texas Board of Nursing.
  • Must be certified CPR & AED and have a clear TB test.
  • Must be able to use reasoning abilities to manage multiple tasks simultaneously and prioritize duties and assignments effectively.
  • Bilingual (English/Spanish) and the ability to read and write in Spanish, strongly preferred but not required.
  • Must be able to relate in a positive and warm manner to seniors, their families, and responsible parties; showing sensitivity to the needs of those living with dementia.
  • Understand and adhere to confidential requirements.
  • Must have intermediate knowledge of office skills including general computer skills, managing electronic records, filing, faxing, copying, and telephone.
  • Must have reliable, personal vehicle and a valid Driver’s License, good driving record, and applicable insurance coverage required by the company policy.

Physical requirements:

  • Have mobility to work in a standard office setting and to use standard office equipment, including a computer; vision to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone.
  • Able to work at a desk and computer screen for extended periods of time; anticipated that the person may spend several hours each day sitting at a computer.
  • Ability to move about freely while assisting clients with tasks, activities, and movement.
  • Ability to work in a highly stressful environment dealing with a wide variety of challenges, deadlines, and diverse employee population.
  • Must be able to react quickly and make decisive and accurate decisions in medical and behavioral situations with clients.
  • Ability to speak with and understand directions and questions in a quick manner. Sometimes with a client that speaks slowly, softly, or with a slurred speech pattern.
  • Ability to assist clients with transfers from sitting to standing and assist with walking.
  • Able to occasionally lift up-to twenty-five (25) pounds to move office supplies or other equipment.
