Job Summary

Duties
- Conduct comprehensive intake assessments for individuals seeking services, including medical history, psychosocial factors, and current needs
- Coordinate with hospital departments such as emergency medicine, ICU, pediatrics, and behavioral health to facilitate patient evaluations and service planning
- Manage electronic health records (EHR) and ensure compliance with HIPAA regulations to protect patient confidentiality
- Collaborate with community resources and social services to support discharge planning and post-acute care arrangements
- Prioritize tasks effectively to handle high-volume intake workflows while maintaining accuracy and attention to detail
- Communicate clearly with patients and their families to explain procedures, gather information, and provide guidance on available services
- Support clinical case management by documenting assessments, updating medical records, and coordinating multidisciplinary interventions
Experience
- Previous hospital experience in a Level I or Level II trauma center setting is highly preferred
- Demonstrated expertise in acute care environments such as ICU, emergency medicine, or inpatient units
- Experience working with children, individuals with disabilities, mental health conditions, or substance use disorders
- Familiarity with medical terminology, diagnostic evaluation processes, and clinical case management practices
- Proficiency in electronic health records (EHR) systems and healthcare data management tools
- Knowledge of community resource coordination within health services social work frameworks
- Strong background in patient assessment, discharge planning, utilization management, and behavioral health support
- Excellent communication skills for working with diverse populations including families and multidisciplinary teams
- Ability to prioritize tasks efficiently in fast-paced hospital or outpatient settings while maintaining high-quality patient service
