Job Description: Pharmacist

Pharmacist Job Summary:

Pharmacists fill orders for medications, monitor patient medication therapy, and provide drug information. Pharmacists supervise and direct support personnel. The pharmacist interacts with patients, family, and other healthcare providers as appropriate to assure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective medication therapy. In the absence of the director of pharmacy, the pharmacist may be required to assume the essential responsibilities and perform the duties of the director of pharmacy.

Pharmacist Shift Availability:

  • PRN (as needed)


Pharmacist Essential Accountabilities:

  • Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications.
  • Provides accurate, adequate, and timely drug information to the professional staff and patients.
  • Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles.
  • Prepares and dispenses medications, including sterile preparations, extemporaneous compounds, parenteral nutrition preparations, and possibly chemotherapy safely and accurately using appropriate techniques.
  • Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians.
  • Prepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area. Must maintain appropriate training and monitoring per current policy.
  • Organize daily operational duties and assist in the supervision of pharmacy technicians, pharmacy aides, students, interns, and other ancillary personnel involved in providing pharmaceutical care.


Pharmacist Department Specific Accountabilities:

  • Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational medication therapy selections.
  • Documents all clinical activities and interventions accurately and completely
  • Participates in the development and presentation of orientation, education, and training programs to the pharmacy personnel, students, medical, nursing, and other staffs.
  • Expected to rotate into departmental call schedule to support after-hours questions or the need to come onsite for emergency dispensing of medications. The call schedule includes 24-hour support over nights, weekends, and holidays to encompass all hours and days through the entire year.

Pharmacist Qualifications:

Education:

  • Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree or Doctorate of Pharmacy degree.


Experience:

  • One to two years of health-system experience preferred, but not required.


Licensure:

  • Currently licensed to practice pharmacy in the state Kansas as designated by the State Board of Pharmacy and in good standing.


Certifications:

  • BLS and ACLS certification required at hire or with the expectation to complete certification through the site with 1 (one) year from hire (start date). PALS certification optional, but preferred.


Skills:

  • Knowledge of the following skills will be needed to effectively complete job duties: word processing (word), electronic mail, fax machine, copier, telephones, personal computer, computer printer, automated dispensing cabinets, torsion or electronic balance.
  • Excellent interpersonal skills demonstrated through interactions within various teams, staff, management, and during presentations.
  • High level of accountability, willingness to learn, and enthusiasm for job performance.
  • Ability to independently and proactively inquire on clinical updates, policies and protocols, and regular face-to-face communication with department manager regarding progress.
  • Must be comfortable with fast-paced, free-flowing work environment with a high degree of autonomy.
  • Autonomy will include the use of independent judgment on clinical and operational decisions, independent work on assigned tasks and projects, and the regular accomplishment of the same with minimal supervision.


Age-Specific Skills:

  • Ability to assist with meeting the pharmaceutical care needs of neonatal, infant, pediatric, adolescent, adult, and geriatric patients (all life stages), as well as special populations.


Pharmacist Working Conditions:

Hazardous Working Conditions:

  • OSHA Category 2: Tasks that involve no exposure to blood, body fluids, tissues, and other potentially infectious materials; but employment may require performing unplanned Category 1 tasks.
  • Potential for exposure to hazardous and toxic substances (potentially chemotherapy, cytotoxic drugs, hazardous drugs, and cleaning solutions). Potential for sticks or cuts by needles and other sharp items. Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.


Environmental Surroundings:

  • Potential exposure to airborne diseases due to the nature of the hospital setting and exposures.


Physical Working Conditions:

  • The environmental surroundings for this position are located in an indoor area with frequent exposure to mild physical discomfort from dust, fumes, temperature, and noise.
  • Repetitive use of hands and fingers (ex. preparing IV admixtures, use of a computer keyboard). May require lifting and carrying light loads (up to 30 lb.), including boxes, equipment, unit-dose cassettes, and IV solutions and stooping or kneeling (ex. to pick up items from the floor, to remove and replace items on lower shelves or top shelves, and to file documents in lower file drawers). Walking or standing for long periods of time (4–8 hours) is often necessary. Must be able to physically operate the equipment used for the job.
