We're hiring experienced LPNs or Social Workers with a bachelor’s degree and at least one year of geriatric experience (think nursing home or home health).
No experience with ADvantage Case Management? That’s okay, we are willing to train the right person. We offer unparalleled flexibility in scheduling!

As an ADvantage Case Manager, you'll be responsible for initiating and monitoring service plans, setting achievable goals, and providing resource referrals to meet the needs of ADvantage Program Members.
