POSITION SUMMARY:
Reports to the DON, the MDS Coordinator (RN) is responsible for coordinating and overseeing the Resident Assessment Instrument (RAI) process, including Minimum Data Set (MDS) 3.0 assessments, Care Area Assessments (CAAs), and interdisciplinary care planning in accordance with CMS, CDPH, and federal regulation §483.20.
A Registered Nurse (RN) must coordinate each assessment with participation of the interdisciplinary team (IDT). The MDS Coordinator collaborates with nursing, therapy, social services, dietary, activities, and business office staff to ensure accurate and timely completion of assessments under OBRA and PDPM.
This role ensures clinical documentation supports MDS coding, reimbursement accuracy, quality measures, and skilled level of care while maintaining compliance and supporting resident outcomes.

POSITION QUALIFICATIONS:
- Current California RN license required.
- BLS certification
- Minimum 2 years SNF/LTC/acute experience
- Knowledge of CMS RAI Manual, PDPM, and Quality Measures
- Strong communication, organizational, and EHR skills
POSITION RESPONSIBILITIES:
- RAI/MDS Coordination: Coordinate MDS 3.0 assessments per CMS RAI Manual
- Ensure timely completion and submission via iQIES
- Review Section Z for IDT attestation
- Sign MDS as RN Coordinator
- Documentation & Coding: Ensure documentation supports acuity and skilled need
- Validate HIPPS coding aligns with clinical documentation and UB-04
- Lead IDT meetings validating diagnoses, function, cognition, payer, and skilled need
- Participate in care planning, baseline care plans, and discharge planning
- Coordinate CAAs and individualized care plans
- Ensure plans reflect current condition and are regularly updated
- Monitor and report Quality Measures, QRP, VBP, and 5-Star metrics
- Participate in QAPI and implement improvement plans
- Maintain knowledge of CMS/CDPH regulations
- Support survey readiness and plans of correction
- Maintain backup MDS coverage plan
- Participate in Medicare meetings
- Audit documentation for MDS accuracy
- Performs other duties as required.
