What are the responsibilities and job description for the RN Homecare Transition Nurse position at Maxim Healthcare?
Hourly Pay: $35 - $37 / Hour
Maxim Healthcare is hiring for Registered Nurses to work as a group to be the initial full-schedule staff for patients transitioning home from the hospital until full ongoing coverage is identified.
Transition Team nurses provide care for a provisional period, between 1-30 days, with a 14 day grace period. During this time, our office staff will be recruiting to identify full-time replacement nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then transitioned to another new admission.

Requirements:
- Minimum one year nursing experience.
- Must have experience with g tubes, tracheostomies, and ventilators or willing to learn.
- Consistent communication with internal staff.
- Ability to maintain a schedule that will change frequently.
- Possibility of lengthy commute times.
- Fortitude to work in varying home environments.
- Must be open to some weekend shifts.
