Matrix Home Care is seeking experienced and compassionate Occupational Therapists (OTs) for PRN home health visits in Port Charlotte, Punta Gorda North, Punta Gorda South, Fort Myers, Cape Coral, Lehigh Acres, and Arcadia, FL. This position is ideal for clinicians who enjoy providing individualized therapy services in the patient’s home and are comfortable working independently while collaborating with the interdisciplinary care team.
The Occupational Therapist will evaluate patients, develop treatment plans, provide skilled occupational therapy services, educate patients and caregivers, and document care in accordance with agency, state, and federal home health requirements.
Responsibilities:
- Complete occupational therapy evaluations and follow-up visits in the patient’s home
- Assess activities of daily living, functional limitations, strength, coordination, cognition, safety, and home environment needs
- Develop individualized treatment plans and goals based on patient condition and physician orders
- Provide skilled therapy interventions to improve independence with daily living activities, mobility, self-care, safety, and functional performance
- Educate patients and caregivers on adaptive techniques, energy conservation, fall prevention, home safety, equipment use, and caregiver support
- Monitor patient progress and update treatment plans as appropriate
- Communicate changes in condition, progress, and recommendations to the care team and physician as needed
- Complete accurate and timely documentation in accordance with home health standards and regulatory requirements
- Maintain compliance with agency policies, AHCA requirements, and applicable Florida home health regulations
Requirements:
- Active Florida Occupational Therapist license
- Prior home health experience preferred
- Current CPR/BLS certification preferred
- Valid driver’s license, reliable transportation, and auto insurance
- Strong assessment, documentation, time management, and communication skills
- Ability to work independently in the home setting
- Comfortable traveling within assigned service areas
