We envision a future where parents impacted by racial and social inequities can birth with dignity, parent with autonomy, and raise babies who are healthy, growing, and thriving.Position SummaryProvides culturally sensitive support, education, and case management services to address the specialized needs of pregnant and newly parenting women with substance use disorders (SUD), their children and their families. Responsibilities include providing assessments, referrals, follow-up, and other required services. Partners closely with major health systems, behavioral health providers, emergency response, and other community based programs in Philadelphia and Montgomery Countiesregion. Essential FunctionsProvides a brief case management intervention to identify needs of women with SUD and help set goals for a healthy prenatal and postpartum experience. Provides assessments,referrals and follow-up support to pregnant women, new parents and their families, through responsive home, community and / or virtual visits and regular phone / text contact.Supports clients in goal setting to plan for their health, social service and recovery needs. Refers participants to longer-term home visiting programs and other appropriate community services for on-going support. Researches and establishes partnerships with community agencies which can support clients. Ensures timely scheduling of prenatal, postpartum, specialist, primary care, well child, and early intervention appointments; accompanies clients to appointmentswhen necessary. Provides patient navigation services to support women with health care needs. Supports women in addressing substance use issues, including providing information on harm-reduction, treatment and MAT options, attending their treatmentprograms and establishing recovery support systems. Assists in organizing client meetings and other special events such as celebrating Mother's and Father's Day. Keeps careful records of all client contacts and follows up as necessary. Routinely uses MCC client database and other computer systems including e-mail, Internet word processing. Attends orientation and ongoing training sessions on maternal and child health and welfare,substance use, housing, nutrition, parenting, child development, infant and family support issues and PA mandatory reporting. Meets with supervisor regularly to review and update on cases. Ensures confidentiality of client records, sharing pertinent information only with the client's approval. Participates in outreach events, health fairs, and other networking opportunities to promote program goals. Other Tasks Adheres to the MCC's security guidelines and ensures the appropriate handling of sensitive information. Attends relevant staff meetings to promote communication and execution of goals. Completes special projects specific to the function of the department or as needed for the department as directed by the supervisor. Other duties as assigned within the scope of position expectations. Knowledge, Skills, and Abilities Understanding of MCC's mission, goals, and objectives and ability to work independently with a high level of energy and contribute as part of a larger team. Knowledge of the concepts and practices associated with community health issues, particularly with pregnant women, prenatal care, parenting and other services for mothers, young children, and their families in the community served. Significant knowledge of and sensitivity to issues facing the community served. Knowledge and skill pertaining to the delivery of substance abuse services. Ability to travel in and around the Greater Philadelphia region to conduct home visits and community outreach. Experience in client management procedures for determining eligibility, assessing needs, identifying resources, making referrals, following up, and documenting client interactions. Strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, community partners, clients, and members of the community. Ability to operate a computer and use a variety of common software programs including Microsoft Office, spreadsheets, and customized databases. Effective and accurate documentation of case notes and client data according to established client management documentation procedures. Ability to organize tasks in an efficient manner and follow-up and follow-through with strong attention to detail.
