Primary Care or Internal Medicine Physician – Concierge Medicine

📍 San Mateo, CA | 💼 Full-Time | 💰 $400,000 – $650,000/year Total Compensation

A leading concierge primary care practice is redefining modern medicine through a high-touch, relationship-driven care model focused on prevention, longevity, and personalized health optimization. This is an opportunity to step away from high-volume medicine and practice in a setting designed around time, depth, and true patient connection.

Physicians in this model function as the “quarterback” of care—leading proactive, data-informed, and highly personalized health journeys.

Practice Model Highlights

• Extremely low panel size (approximately 2–4 patients actively managed at a time in care cycle model per day)
• Extended visit times designed for deep clinical engagement
• Intake visits: 2–3 hours per patient
• Follow-ups: 30-minute telehealth and 60-minute in-person visits
• Hybrid care model with flexible remote touchpoints as needed (daily or PRN)
• No injection-based procedural requirements
• Focus on prevention, longevity, and functional optimization over reactive care

Key Responsibilities

• Lead comprehensive, relationship-based care for a small, high-touch patient panel
• Conduct extended intake visits to fully assess medical history, lifestyle, goals, and risk factors
• Develop and manage individualized preventive and longevity-focused care plans
• Integrate advanced diagnostics including genomics, imaging, wearables, and comprehensive lab panels
• Coordinate interdisciplinary care across specialists and health advisors
• Provide ongoing longitudinal care with a focus on outcomes, prevention, and optimization
• Educate and empower patients through lifestyle interventions (nutrition, sleep, stress, exercise)
• Participate in clinical protocol development and innovation in preventive medicine
• Leverage technology (AI tools, EMRs, remote monitoring) to enhance efficiency and insight
• Maintain strong communication and continuity across care team members

Ideal Candidate Profile

• MD or DO with completion of residency in Internal Medicine, Family Medicine, or Emergency Medicine in US Only.
• Board certified or board eligible
• Minimum 5 years post-residency experience preferred
• Strong background or interest in preventive medicine, longevity medicine, or functional medicine
• Experience in concierge medicine or direct primary care strongly valued
• Excellent communication skills with a patient-centered, empathetic approach
• Comfortable using modern digital health tools, wearables, and data-driven care models
• Passion for building meaningful patient relationships and redefining traditional primary care
