Mat‑Su Health Services, Inc. (MSHS) is seeking a mission‑driven Community Health Worker to join our Primary Care team. This role is ideal for someone who is passionate about helping underserved populations, enjoys community engagement, and thrives in a collaborative healthcare environment.

The Community Health Worker I works under the indirect supervision of the Primary Care Clinic Manager and collaborates with interdisciplinary FQHC teams to provide Social Determinants of Health (SDOH) assessments, short‑term care coordination, patient education, and linkage to community resources.

 

This role also represents MSHS at health fairs, resource events, and community outreach activities.
 

Qualifications:

Essential Duties & Responsibilities

  • Conduct outreach and engage eligible patients to identify SDOH factors impacting health and well‑being.

  • Document all patient encounters, assessments, ROIs, service plans, and outcomes in the electronic health record (EHR).

  • Coach patients in self‑management of health conditions and care plans.

  • Assist patients in accessing medical, behavioral health, and community‑based services.

  • Guide patients through the PRAPARE screening process and ensure timely documentation and coding.

  • Maintain knowledge of PCMH, UDS, CARF, and care coordination requirements.

  • Expand knowledge of community resources and facilitate patient access to appropriate services.

  • Provide Narcan training after completing required agency training.

  • Support front desk operations and assist patients with check‑in processes as needed.

  • Attend health, resource, and job fairs.

  • Monitor and report on CHW grants quarterly or as appropriate.

Minimum Qualifications

  • High school diploma or equivalent.

  • 2–5 years of customer service experience; healthcare setting preferred.

  • Must obtain Community Health Worker (CHW) and Health Coach certification within 6 months of hire.

  • Clear background check (DHSS fingerprinted).

  • Current driver’s license and clear driving record.

  • Current TB screening test result.

  • Current First Aid and CPR certifications; CPI certification required when offered.

Knowledge, Skills & Abilities

  • Strong verbal and written communication skills.

  • Ability to independently plan, schedule, and conduct activities.

  • Proficiency with standard office equipment and word‑processing software.

  • Ability to work collaboratively within interdisciplinary teams and community networks.

Working Conditions

  • Work performed in clinic settings and various community locations, including patient homes.

  • Frequent driving required.

  • Low‑risk hazard exposure.

  • MSHS is a non‑smoking campus.

Physical Demands

  • Sitting, standing, bending, reaching, lifting under 50 lbs., speaking, and driving.

  • Manual dexterity to operate office equipment.

  • Normal hearing and vision acuity required.
