- Associate degree in Physical Therapy from an accredited program.
- Current Oregon PTA license and active state license required.
- Home health experience and CPR certification.
- BLS certification may be required.
- Provide therapy sessions Monday to Friday during standard working hours.

- Ensure quality patient care and progress tracking.
