- Performing patient evaluations and developing individualized plans of care.
- Documenting progress and discharge summaries in compliance with state and CMS standards.
- Coordinating care with physicians and interdisciplinary teams.
- Traveling within the service area and to nearby locations as needed.
- Participating in staff meetings and maintaining professional licensure and competencies.
- Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
- Active Arkansas physical therapy license or ability to obtain licensure.
- Home health and OASIS experience required.
- BLS/CPR certification.
- Ability to complete patient assessments, develop care plans, and document appropriately.
- Experience collaborating with physicians and teams.
- Willingness to travel within the service area.
- Experience with HCHB electronic documentation system.
- Previous supervision of PTAs or home health aides.
