Key Responsibilities Include

• Collaborating with physicians and the clinical team to develop, implement, and revise therapy treatment plans addressing daily living skills, community reintegration, adaptive equipment, and physical functions.
• Participating in care coordination and discharge planning, serving as a resource for healthcare personnel.
• Training staff and family members in managing therapy-related activities such as dressing, feeding, grooming, and hygiene.
• Submitting evaluations, treatment plans, progress reports, and discharge summaries.
• Providing clear instructions to Occupational Therapy Assistants and Home Health Aides, and overseeing their performance and documentation.
  
  

Qualifications And Skills Required

• Unrestricted Occupational Therapy license in the relevant state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
  
  

Preferred experience: Home health experience is advantageous.

Compensation: $54.00 - $76.00 per visit/unit, with an annual base pay range of $85,400 - $117,500.

Additional requirements: Valid state driver’s license, proof of vehicle insurance, TB screening, and adherence to safety and health protocols.
