Key Responsibilities Include
- Collaborating with physicians and the clinical team to develop, implement, and revise therapy treatment plans addressing daily living skills, community reintegration, adaptive equipment, and physical functions.
- Participating in care coordination and discharge planning, serving as a resource for healthcare personnel.
- Training staff and family members in managing therapy-related activities such as dressing, feeding, grooming, and hygiene.
- Submitting evaluations, treatment plans, progress reports, and discharge summaries.
- Providing clear instructions to Occupational Therapy Assistants and Home Health Aides, and overseeing their performance and documentation.
- Unrestricted Occupational Therapy license in the relevant state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
