- Observe and report changes in patient or family condition, documenting findings and notifying supervisors.
- Administer treatments and medications per the plan of care and state regulations.
- Educate patients and caregivers on self-care techniques.
- Communicate patient status updates to the care team, including case managers and physicians.
- Conduct supervisory visits for LPNs and Home Health Aides, ensuring competency and providing instructions.
- Maintain accurate data collection, reporting, and documentation systems.
- Identify and report potential risks, abuse, or unsafe practices as required by law.
- Participate in multidisciplinary case conferences, staff meetings, and educational programs.
- Coordinate care with patients, families, and healthcare team members.
- Respond to on-call, after-hours, and weekend needs as necessary.
- Ensure confidentiality and comply with regulations such as Medicare, Medicaid, and HIPAA.
- Adhere to policies, procedures, and standards, performing additional duties as assigned.
- Graduate of an accredited registered nursing program.
- Licensed as an LPN in the practicing state.
- Experience in clinical settings such as ICU, pediatrics, rehab, or med-surg; home health experience preferred.
- Ability to conduct home visits and provide nursing care in home environments.
- Proficient in documentation, communication, and interpersonal skills.
- Reliable transportation, valid driver's license, and insurance or access to public transit.
