Summary:
- Mass Care Link, Inc is currently seeking an experienced per diem Care Manager to conduct monthly and bi-monthly home visits to ensure continuity of care of our current members and support their respective caregivers. This position is an integral part of our multidisciplinary team.
Responsibilities:
- Call and schedule monthly and/or bi-monthly home visits with members and caregivers assigned to you.
- Conduct on-site home visits in the member and caregiver’s home.
- Complete required documentation including but not limited to progress notes and timesheets daily.
- Review caregiver’s documentation of assistance and additional updates/notations for accuracy and timely submission.
- Provide appropriate follow up throughout the month on a case-by-case basis.
- Complete all other required documentation.
Requirements:
- Bachelor’s Degree
- Driver's License
- Reliable vehicle
- 2 years recent experience working with elderly or disabled adults
- Must be able to pass (a) CORI and SORI background checks
- Must provide a driving record
- Must be available for initial orientation/training during the hours of 8:30-4:30, Monday through Friday
- Must be available for ongoing required quarterly meetings/trainings (quarterly)
- Must be available to communicate (via text, email, and or phone call) with agency staff between the hours of 8:30AM and 4:30PM, Monday through Friday.
