About Nova Leap Care Management

Nova Leap Care Management is a private-pay, concierge care management service designed to support individuals and families navigating complex medical, social, and life transitions. Our approach is highly personalized and relationship-driven. We partner closely with families, healthcare providers, and community resources to ensure individuals receive the right care, at the right time, in the right setting. At the core of our work is a simple belief: people deserve to feel supported, informed, and confident during life’s most complex moments.
