Position Summary
The Clinical Case Manager plays a critical role in delivering high-quality, client-centered care management services. This position is ideal for an experienced clinician who thrives in a dynamic, non-traditional environment and is passionate about advocacy, problem-solving, and building meaningful relationships with clients and families. This is not a desk-based role. Clinical Case Managers are actively engaged in the community-meeting clients in their homes, attending appointments, coordinating services, and navigating complex systems in real time. The ideal candidate is both clinically strong and highly resourceful, with the ability to operate independently while maintaining close collaboration with leadership.
Key Responsibilities
Clinical Assessment & Care Planning
- Conduct comprehensive biopsychosocial assessments of clients, including medical, functional, emotional, and environmental factors
- Develop individualized, client-centered care plans with clear, actionable recommendations
- Continuously reassess needs and adjust care plans based on changes in condition or goals
Care Coordination & Implementation
- Coordinate and oversee services across the continuum (home care, assisted living, rehabilitation, skilled nursing, etc.)
- Facilitate transitions of care and ensure safe, appropriate discharge planning
- Partner with healthcare providers to ensure alignment with treatment plans and follow-through on recommendations
Client & Family Support
- Build strong, trusting relationships with clients and families
- Provide education, guidance, and emotional support during complex or stressful situations
- Navigate family dynamics with professionalism, empathy, and clear communication
Advocacy & System Navigation
- Advocate for clients across healthcare, insurance, and community systems
- Identify and address barriers to care, including access, safety, and financial considerations
- Serve as the central point of contact for care-related decision-making
In-Person Engagement
- Accompany clients to medical appointments as needed
- Conduct home visits to assess safety, care needs, and environmental factors
- Support real-time problem solving in the field
Documentation & Operations
- Maintain timely, accurate documentation of all client interactions and services
- Track time and services in accordance with private-pay billing structure
- Utilize EMR and technology platforms effectively
Collaboration & Growth
- Participate in supervision, team meetings, and case reviews
- Contribute to the development of processes, resources, and best practices
- Engage in ongoing professional development
Qualifications
- Active licensure in Massachusetts (LICSW, LCSW, RN, PT, OT, SLP, or equivalent)
- Master’s degree preferred (Social Work, Nursing, or related clinical field)
- 5 years of experience in case management or related clinical roles
- Experience working with older adults, complex medical needs, or family systems strongly preferred
