Position Summary

The Masonic Home of Virginia is seeking a compassionate and organized Social Worker to support residents across multiple levels of care within our Continuing Care Retirement Community. This position serves as a key advocate for residents and families, providing psychosocial support, coordinating care transitions, and collaborating with an interdisciplinary team to ensure each resident receives person-centered care.

Key Responsibilities

• Provide counseling, emotional support, and crisis intervention to residents and families.
• Serve as a liaison between residents, families, healthcare providers, and community agencies to facilitate communication and resolve concerns.
• Coordinate resident transitions between Independent Living, Assisted Living, Skilled Nursing, and other levels of care.
• Schedule and lead quarterly care conferences for Skilled Nursing residents.
• Develop, coordinate, and maintain UAI’s and Individualized ISPs for Assisted Living residents, including annual reviews.
• Complete admissions, assessments, progress notes, and other required documentation in accordance with state and federal regulations.
• Coordinate pre-admission assessments and prepare case studies for prospective residents.
• Manage the community’s Outreach Program by working with applicants, Area Agencies on Aging, and the finance department to coordinate services and financial assistance.
• Assist residents and families with discharge planning, end-of-life support, and bereavement resources.
• Maintain accurate resident records within MatrixCare EMR and complete required quarterly MDS social service documentation.
• Prepare documentation and reports for Board meetings and other organizational needs.
• Participate as an active member of the interdisciplinary care team, collaborating with nursing, therapy, activities, dining, admissions, and other departments to meet resident needs.

Education

• Four-year Degree in Social Work or related field

Qualifications

• Bachelor’s Degree in Social Work or a related human services field (BSW preferred).
• Minimum 2 years experience in skilled nursing, assisted living, rehabilitation, hospice, or senior living preferred.
• Knowledge of psychosocial assessments, care planning, and long-term care regulations.
• Strong communication, organization, and problem-solving skills.
• Experience with electronic medical records (MatrixCare experience preferred).

Physical Requirements

• Ability to stoop, kneel, crouch, reach, walk, push, pull, lift, and move about in the work area and throughout the Home to accomplish tasks

• Express and exchange ideas via spoken word conveying information to others in an audible, accurate, and quick manner

• Perform repetitive motions with wrists, hands, and fingers

• Visual acuity to read, prepare, and analyze

• Must be able to exert up to 20 pounds of force on occasions

• Subject to temperature changes, but is generally not substantially exposed to adverse environmental conditions as the work is predominantly inside

Salary : $58,000 - $65,000
