Full-time 2:45pm - 10:45pm

Job Functions:

Clinical/Support Activities

1. Assesses the physiological, psychosocial and spiritual needs of residents and their

support system, and documents on the nursing care plan.

Assesses the physical and emotional needs upon admission, and throughout stay.

Recognizes and documents changes in resident condition.

Distinguishes between age related physical and psychosocial changes, and those changes indicative of illness and disease.

2. Assesses and plans for the learning and potential discharge needs of residents and families.

Assesses the learning and discharge needs on admission and throughout stay.

Includes family’s input in educational and discharge plans.

Formulates a nursing diagnosis that accurately reflects the assessment of the resident’s status.

Documents outcomes for each nursing diagnosis.

Utilizes appropriate resident education resources.

3. Collaborates with resident, family and health team to establish resident outcomes

and make modifications as necessary.

a. Uses feedback from physician, resident, family and other health care team

members to access resident’s ongoing progress.

b. Identifies interventions for resident, family, nursing and ancillary staff; and

selects appropriate interventions and documents any changes.

c. Notifies physicians of pertinent observations and reactions.

d. Completes required nursing documentation to include MDS and MDS .

4. Establishes, in collaboration with the physicians, resident, family and ancillary

staff; treatment priorities to formulate and document a plan of care.

a. Plan of care should reflect the current physical and mental status of resident,

and be based on nursing and medical diagnosis.

b. Document specific nursing interventions stating expected outcomes based upon

age and abilities of resident.

c. Review and/or transcribe physician orders accurately.

d. Contacts physician when there is a question concerning orders or resident condition.

e. Accompanies physician on rounds and assists with examinations and treatments as needed.

5. Performs nursing interventions and documents the intervention appropriately.

a. Administers ordered medications and treatments correctly and safely.

b. Observes and documents resident’s response to medication and treatments.

c. Assesses residents on ongoing basis to detect changes in residents’ conditions.

d. Provides basic emergency care.

e. Assists with activities of daily living.

f. Takes and records vital signs and responds appropriately with changes.

6. Provides direction, assistance and monitoring of Nursing Care.

a. Delegates nursing functions based on resident needs.

b. Makes rounds at beginning of shift to assess and validate report information.

c. Communicates changes in practice or delivery of care.

7. Provides a safe and secure environment for the resident

a. Assures resident call bells are within reach and responds to call bells in a timely manner.

b. Assesses resident condition to determine what safety precautions are necessary.

c. Establishes a therapeutic environment for resident and family.

d. Demonstrates appropriate steps in identifying, replacing and requesting repairs to defective resident/transport equipment.

e. Prepares resident for and assists in transfers utilizing appropriate body mechanics to protect self during resident transfers.

f. Washes hands after each resident contact.

g. Utilizes appropriate protective barrier equipment.

h. Disposes of sharps according to policy.

i. Directs ancillary personnel regarding appropriate precautions.

j. Directs and may assist with department cleaning and are of equipment and

appliances.

8. Performs the evaluation of resident’s progress towards identified outcomes, and

makes modifications to plan of care accordingly.

a. Collaborates with other health team members, resident and family in the

Evaluation or modification of goals.

b. Documents changes on care plan.

c. Notifies physician of changes.

d. Documents family understanding of modifications or outcomes, and teaching

and learning.

e. Makes in-house referrals as appropriate.

9. Performs other related duties as assigned or directed.

Quality Assurance Activities

Actively participates as a member of the department in improving quality of care/service.

a. Identifies ways to improve care.

b. Participates and encourages a focus on continuous quality improvement.

c. Identifies and corrects safety hazards within individual control. Reports all

others to appropriate department for correction.

d. Responds to fire drills and disaster drills.

e. Uses products, supplies and equipment in the most efficient and cost effective way.

Professional Development/Educational Activities

a. Attends mandatory education programs on fire, safety, infection control, etc.

b. Identifies own learning needs and communicates to Nurse Manager.

c. Attends educational/training programs that will enhance/increase knowledge and skills relative to performance expectations.

d. Maintains confidentiality regarding resident information and other sensitive issues.

e. Seeks involvement in committees and professional organizations.

f. Assists in orientation of new personnel.

g. Serves as professional role model.

h. Performs duties as related to the department as requested, including Charge Nurse function.

JOB QUALIFICATIONS:

Graduation from an accredited school of nursing and current license by State Board of Nursing.

Must pass Medication Testing and Skills Proficiency on annual basis.

The base range for this position is $38 - $40 per hour commensurate with work location, experience, skills, certifications, education & prior accomplishments.

The position may be eligible for additional compensation not listed here, including but not limited to shift differentials, incentives or bonuses, as applicable.
