SUMMARY:

Masonic Care Community is seeking a dedicated Physical Therapist to join our dynamic interdisciplinary team in our skilled nursing facility with a strong short term rehabilitation program as well as outpatient therapy. The PT will complete thorough evaluation, treatment and discharge planning for individual’s recovery from injury, illness or surgery as well as our long term care residents requiring restorative therapy services. The ideal candidate should be committed to provide evidenced based, resident-centered care with emphasis on positive functional outcomes.

RESPONSIBILITIES:

· Conduct comprehensive evaluations for residents in short term rehab, long term care and outpatient rehab.

· Develop individualized, resident-centered and goal oriented treatment plans with emphasis on improving strength, mobility, balance, functional activity tolerance and overall independence.

· Deliver skilled interventions including, therapeutic exercise, therapeutic activities, gait training. Neuro-muscular re-education, pain management and modalities as appropriate.

· Work closely with the interdisciplinary team to ensure positive functional outcomes.

· Provide education and training to Residents, families/caregivers and staff on exercise programs, equipment use, functional mobility status and fall prevention strategies.

· Participate in departmental and facility meeting as well as care planning meetings.

· Supervise and provide guidance to Physical Therapist Assistant(s).

· Maintain timely, compliant and accurate documentation in accordance with facility, state and federal guidelines.

· Active participation in staff development, continuing education and best practice within the rehab department and nursing facility.

· Assist with education inservices for staff training as needed.

· Assist with maintaining a safe, clean and productive therapy department.

· Maintain appropriate productivity standard and adhere to departmental expectations for compliance and timeliness of documentation as well as resident care delivery.

QUALIFICATIONS:

· Graduate of an accredited Physical Therapy program with a Doctorate, Masters or Bachelor’s degree in Physical Therapy.

· Current Physical Therapist license in the state of New York.

· Prior experience in long term care and short term rehab preferred, however, new graduates are welcome.

· Strong clinical reasoning and problem solving skills.

· Strong interpersonal and communications skills are a must with residents, families and the interdisciplinary team.

· Excellent organizational skills and time management skills and the ability to work in a fast paced rehab department.

· Committed to providing quality outcomes with emphasis on resident centered care.

· Familiarity with electronic medical record systems.

The base salary range for this position is $85K - $92,600k per year commensurate with work location, experience, skills, certifications, education & prior accomplishments.

The position may be eligible for additional compensation not listed here, including but not limited to shift differentials, incentives or bonuses, as applicable.
