Primary Function:
The Home Health RN provides professional nursing care to patients in their homes, ensuring the highest standards of care are met. This role involves assessing, planning, implementing, and evaluating patient care while working collaboratively with a multidisciplinary team to support patients' health needs and promote recovery in a home setting.
Responsibilities:
- Provide direct nursing care to patients in their homes, including but not limited to medication management, wound care, vital signs monitoring, and patient education.
- Perform physical assessments to evaluate patient conditions and needs.
- Develop and implement individualized care plans based on patient needs, in consultation with other healthcare providers.
- Educate patients and families about disease processes, self-care techniques, and medication management.
- Monitor patients for any changes in health status and report findings to the provider or healthcare team.
- Maintain accurate and timely documentation of patient care, assessments, treatment plans, and progress notes in compliance with agency policies and regulatory standards.
- Ensure all patient care activities are documented in the Electronic Health Record (EHR) system.
- Collaborate with providers, physical therapists, occupational therapists, and other healthcare professionals to coordinate care.
- Communicate effectively with patients and their families to ensure understanding and adherence to care plans.
- Provide support to family members regarding patient care needs and education on managing medical conditions at home.
- Foster a professional and confidential relationship with staff and patients, encouraging open communication, trust, and a high standard of professionalism within the department and the hospital.
- Performs other related duties as assigned to meet the needs of the organization.
- Monitor patient progress and adjust care plans as needed to achieve desired outcomes.
- Provide regular updates to the case manager, clinical manager or director of nursing regarding the patient’s status and any concerns.
- Ensure proper documentation and compliance with home health regulations.
- Adhere to all healthcare protocols, safety regulations, and privacy guidelines (HIPAA).
- Follow infection control practices and standard precautions to ensure patient safety.
- Ensure that equipment and supplies used for patient care are well-maintained and properly sterilized.
- Respond promptly to any emergency situations, provide first aid or basic life support if necessary, and ensure timely transportation to the hospital if required.
- Foster a professional and confidential relationship with staff and patients, encouraging open communication, trust, and a high standard of professionalism within the department and the hospital.
- Performs other related duties as assigned to meet the needs of the organization.
Qualifications:
- Graduate of an accredited nursing program.
- Valid Registered Nurse (RN) licensure in Illinois.
- Minimum of 1 to 2 years of nursing experience in a healthcare setting.
- Experience in wound care, chronic disease management, and/or geriatric care is a plus.
- Strong clinical assessment skills and ability to make independent nursing decisions.
- Ability to develop, implement, and evaluate individualized care plans.
- Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
- Ability to work independently and manage time effectively while caring for multiple patients.
- Basic Life Support (BLS) certification required.
- Able to obtain Dementia training as required by IDPH.
Physical Demands/Work Environment:
- Ability to lift 20 to 30 pounds and move patients with assistance as needed.
- Frequent walking, standing, and bending to perform patient care activities.
- Ability to transport equipment and supplies for patient care.
- This position involves traveling to patients’ homes within the assigned geographic area, must be able to provide own reliable transportation and proof of automobile insurance.
- Flexible working hours, including the possibility of weekend and evening shifts, based on patient needs.
