- Coordinate specialty and hospital care while staying actively involved
- Lead and collaborate with an interdisciplinary care team
- Supervise and remain accountable for Advanced Practice Providers (APPs)
- Partner with medical and operational leadership to meet quality and cost goals
- Hospital admissions per thousand
- Appointment utilization (18–21 slots per day)
- Patient experience (CG-CAHPS)
- Closure of clinical care gaps
- Medical cost management
- MD or DO required
- Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
- Active or eligible state medical license
- Active DEA
- Commitment to value-based, team-oriented care
Compensation & Benefits
