Job Summary: The Admissions Coordinator/QIDP is responsible for overseeing and managing the residential admission process for all new admissions, internal transfers, and program discharges within the agency. This role ensures that all transitions are coordinated effectively, with clear communication among internal teams, external facilities and regulatory agencies. In addition to admissions responsibilities, the Admissions Coordinator/QIDP maintains an active caseload of Maryhaven residential individuals, ensuring that all individualized service plans comply with regulatory standards and certification requirements.
Job Requirements:
- Bachelor’s degree in human services field required and two years experience with the ID/DD population.
- One year of OPWDD Residential Manager experience preferred.
- Knowledge of regulatory agency requirements, and auditing procedures preferred.
- Computer literate. Able to learn new programs as needed.
- Knowledge of Choices, and Precision Care software, preferred.
- SCIP Recertification preferred; AMAP certification preferred.
Job Responsibilities:
- Ensure the coordination of all facets of the admission & internal transfer process including soliciting and reviewing packets, scheduling of screenings/tours, parent and intra/interagency notifications, necessary documentation/tracking and follow up.
- Coordinate all Admission meetings including new admissions, internal transfers, discharges to alternate agency, etc.
- Completes all required notifications and submits all admission/discharge paperwork to LIDDRO and MHLS as required.
- Ensures that admission/discharge policies are current and adhered to by program staff and families.
- Manage organized file systems for applicants for those who are screened and deemed appropriate candidates for residential placement, internal transfers and discharges.
- Maintains phone contact with families and LIDDRO staff.
- Completes programmatic reports as required.
- Submit/review DDP1’s upon admission and discharge, and DDP2’s every two years as applicable.
- Serve has a OPWDD Choices resource for residential department.
- Performs all other required tasks as requested by Director of Residential Operations and Admissions.
- Functions as a member of the interdisciplinary team, including scheduling meetings and keeping all members appraised of pertinent issues.
- Participates in Annual/Semi-Annual/30-day meetings, ITT’s, intake and discharge meetings, and ensures written documentation is of professional quality and completed in a timely manner and forwarded to correspondents within regulatory guidelines.
- Work closely with residential management and site Program Manager to ensure all monthly and annual requirements comply and of quality as per regulation.
- QIDP required to ensure compliance in all areas of residential habilitation as per Medicaid requirements including certification of monthly notes, semi-annual notes, certification and distribution of Staff Action Plans.
- Promotes and ensures individual choice and advocates for inclusion in decision-making.
- Visits individuals on a regular basis to ensure services are in alignment with agency goals and philosophy
Pay: $30.00 per hour
