JOB DEFINITION:Responsible for processing referrals and maintain occupancy rates. Perform outreach to referral sources. Ensure/Confirm benefits for prospective new residents. Complete intakes for prospective new residents.EDUCATIONAL REQUIREMENTS:Bachelor's degree in human services and minimum of 2 years' experience in health/ mental health related field. Masters in Social Work or Mental Health Counseling degree preferred.EXPERIENCE REQUIRED: Two years of experience in a mental health setting.DUTIES AND RESPONSIBILITIES:Monitor program occupancy rates, manage the CAIRS system, and ensure that all efforts are being made to achieve maximum occupancy;Review all applications for completeness and complete all necessary follow-ups prior to and post screeningMaintain open lines of communication with the Single Point of Access, County and State representatives regarding the admissions process;Ensure that all paperwork and reporting documents required by the Single Point of Access and Office of Mental Health are completed accurately and in a timely manner;Attend Single Point of Access meetings, as scheduled, and represent agency in these forums;Ensure that processing of all referrals is done within 24-48 hours and in compliance with regulatory requirements including maintaining the CAIRS system, the SPA portal, and ensuring all referral documentation is uploaded into the EHR;Coordinate and conduct in-person intake interviews;Maintain communications with the SPA liaison the referring worker and prospective resident throughout the application process;Coordinate the presentation of applications to the Admission/Discharge Committee and complete agency documentation as per Admission procedure;Ensure residents have active Medicaid in place prior to admissions to all programsRun EpacesContact treatment teamLinkage to Case ManagementEnsure that Physician's Authorization forms are in place prior to admissions to licensed housing programs and Documentation of Disability for unlicensed housing programs;Coordinate overnight visits and move-in dates with program supervisors and administrative staff;Oversee the work flow regarding all aspects of the admission process and coordinate with Assistant Director for all changes/updates that need to be made for this process;Facilitate (2) weekly admission and movement team meetings to implement and coordinate site vacancies and preparedness for movement;Attend all in-person management meetings and other required in-person meetings/trainings.Fieldwork is required 3-5 times weekly, and as needed.Ensure all Psyckes consents are uploaded into the Psyckes system and manage the tracking system for these consents.Identify and implement quality improvement trainings and activities as part of professional development goalsEncourage quality improvement processes and provide consistent resources for QI and professional developmentAbility to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
