What are the responsibilities and job description for the LPN - Home Health position at Mary T Inc.?
The Mary T Family of Companies is hiring for a full-time Licensed Practical Nurse (LPN) in Coon Rapids, MN and surrounding cities.
- Full-time hours would be 4 - 5 days per week (30-32 hours)
- Provide skilled nursing care as directed by the RN Case Manager and/or RN Clinical Nursing Manager.
- Promotes and maintains patient health through teaching appropriate rehabilitative measures under the guidance of a registered nurse.
- Assists patients in learning appropriate self-care techniques.
- Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals.
- Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.
- Communicates current health status and needs of patients to appropriate health professionals.
- Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks.
- Documents appropriately and timely in Home Health software system.
- Provides and maintains a safe and clean environment for the patient per organization standards.
- Responsible for following all policies and procedures of Home Health agency.
- Accountability for the quality of care delivered and performing to the level of education, knowledge, and skill ordinarily expected of an individual who has completed an approved practical nursing education program
- LPN Degree, from an accredited nursing program
- Active LPN license in MN, and in good standing.
- Valid driver’s license.
- One year previous LPN experience in a Home Health agency, preferred.
- Basic Computer skills
- Strong communication skills
- Good organizational skills with ability to prioritize
- Above average self-discipline, and the ability to self-direct
- Ability to adapt to changing environments
- Good level of patience with the patients and employees
- Strong problem-solving skills
- Good working knowledge of illnesses and disabilities
- Knowledge of patient rights and HIPAA compliance
- Knowledge of state and federal regulations
- Knowledge of and ability to practice safe patient handling
