Schedule: Full-Time | Monday-Friday On-Call and Weekend Rotation
Compensation: $30-$38 per visit Overtime Opportunities

What You'll Be Doing
- Complete skilled nursing visits and ongoing assessments
- Administer medications and treatments according to physician orders
- Monitor changes in condition and communicate updates to the care team
- Educate patients and families on disease management and safety
- Promote comfort, healing, and independence
Be A Trusted Partner
- Collaborate closely with RN Case Managers and interdisciplinary teams
- Support individualized care plans
- Maintain accurate documentation
- Help prevent unnecessary hospitalizations and improve outcomes
What You Bring
Required
- Active Minnesota LPN license
- Minimum one year of clinical experience
- Strong assessment and communication skills
- Ability to work independently with confidence
- Reliable transportation and willingness to travel
Preferred
- Home health, hospice, TCU, or long-term care experience
- Passion for relationship-based nursing
