Physical Therapist - Skilled Nursing Facility
Hours Available:
40 hours/week - 1-2 weekend shifts per month
Mary Ann Morse Healthcare Center, a 118 bed skilled nursing facility located in Natick, MA, is looking for a Physical Therapist to join our team. Summary
To provide patient evaluation, care planning and direct treatment to a general rehab caseload. Participates in the clinical team process. Provides teaching and training to patients, families and caregiver staff. Participates in clinical program development.
Essential Job Functions: Responsibilities and Standards
- Screens patients according to policy.
- Evaluates patients and develops appropriate plan of treatment.
- Under a physicians order, provides evaluation and direct patient treatment in accordance with an established plan of care.
- Contributes to the interdisciplinary team meeting/process.
- Consults with and trains patients, families and caregivers.
- Maintains productivity expectations on an ongoing basis.
- Documents treatment according to company’s clinical documentation system on a timely basis.
- Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations.
- Communicates effectively with patients, families, caregivers, team members and supervisor.
- Promotes positive customer relations.
- Participates in clinical program development and implantation.
- Demonstrates ability for continuous new learning to remain current in the field.
- Participates in quality improvement activities.
- Exhibits courtesy, compassion and respect to patients, families, caregivers, visitors, physician, administrators and other team members.
- Delivers services in compliance with the facilities ethical standards, policy and procedure, national professional standards, state practice acts where applicable, and current accepted state of the art practice.
- Other: Tasks and duties as assigned by the Rehabilitation Manger or Administrator as needed.
Qualifications:
Knowledge, Skills, Experience Required
- Bachelor of Science , Master of Science or Doctorate Degree in Physical Therapy and current professional licensure in the state of hire.
- At least one year of work experience as a Physical Therapist working with adults.
- Orthopedic and manual skills are preferred.
- Possess strong oral and written communication skills, Organizational and analytical skills required.
- Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers.
- Demonstrated ability to utilize independent and effective clinical judgment, problem-solving and decision-making skills.
- Demonstrates effective time management skills.
