Job Responsibilities:
- Provides for safe and effective use of medications through comprehensive Medication Therapy Management and consults with members of the healthcare team.
- Works with inpatient clinical pharmacy to provide Transitions of Care services for Internal Medicine patients at discharge until hospital follow-up appointment in clinic.
- Participates as a member of the Patient Centered Medical Home team to assist in achieving excellent Medicare Star measure ratings and other metrics as deemed necessary.
- Manages patients and disease states per Collaborative Pharmacy Practice Agreement including, but not limited to: Diabetes, Hypertension, and Hyperlipidemia.
- Provides clinical consultation to practitioners such as appropriate, cost-effective therapeutic alternatives through knowledge of the 340B pharmacy program, obtaining Prior Authorization for specialty medications, and referral of patients to Marshall Pharmacy when appropriate.
- Provides clear and concise communication with patients, clinicians, and medical providers.
- Prevents, detects, and resolves medication-related problems and makes appropriate, patient-centered medication therapy recommendations.
- Counsels patients and caregivers on medication use and disease management, safe administration measures, and side effect management strategies.
- Teaches, mentors, and precepts pharmacy/medical students and residents.
- Performs additional duties as assigned.
Education: Must hold a valid and current Pharmacist's License in the state of West Virginia or ability to obtain licensure in the state of West Virginia prior to hire date.
Experience: PGY-1 Pharmacy Residency or Board of Pharmacy Specialties (BPS) Certification preferred. Minimum 2 years experience in clinical ambulatory patient care including management of diabetes, hypertension, and hyperlipidemia preferred.
