The Valley Springs Health & Wellness Center team is looking for a Care Management Coordinator, RN to create and maintain a Chronic Care Management (CCM/PCM) program within the clinic.This role involves supporting the current nursing staff to ensure quality patient care and effective management of chronic conditions.
- Develop and implement a comprehensive Chronic Care Management program tailored to the needs of the clinic's patients.
- Collaborate with nursing staff to enhance patient care and streamline care management processes.
- Monitor and evaluate patient progress within the CCM program, making adjustments as necessary to improve outcomes.
- Provide education and support to patients and their families regarding chronic condition management.
- Work closely with healthcare providers to coordinate patient care and ensure continuity across services.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
- Participate in team meetings and contribute to the development of clinic policies and procedures related to chronic care management.
- Serve as a resource, offering guidance and support in managing complex patient cases.
- Serves as a flexible clinical resource providing pateint triage, nursing guidance and support to Medical Assistants within scope of practice, and maintaining cointinuity of nursing services through cross-coverage as needed

- Associate's degree in Nursing with an active California RN license
Preferred Education:
- Bachelor's degree in Nursing preferred
Required Experience:
- Experience in nursing with a focus on chronic care management
- Familiarity with creating and maintaining care management programs
- Ability to support and collaborate with current nursing staff
Preferred Experience:
- 3 years in nursing with Med-Surg, ICU, or chronic condition management experience preferred
Required Skills and Abilities:
- Strong organizational skills to manage and maintain care programs
- Excellent communication skills to effectively support nursing staff
- Ability to work independently and as part of a team
- Proficiency in using healthcare management software and tools
Salary : $45 - $55
