Position Summary:
We are seeking a compassionate and experienced Social Services Director to lead our social services department. This role is responsible for overseeing the psychosocial well-being of residents, coordinating discharge planning, and serving as a vital liaison between residents, families, and the interdisciplinary care team.

Key Responsibilities:

• Assess residents’ psychosocial needs and develop individualized care plans
• Provide emotional support and counseling to residents and families
• Coordinate discharge planning and community resource referrals
• Advocate for residents’ rights and ensure their needs are met
• Maintain accurate documentation in compliance with state and federal regulations
• Collaborate with nursing, therapy, and administrative teams to support holistic care
• Supervise and support social services staff, if applicable

Qualifications:

• Current and valid Licensed Social Worker (LSW) license in the state of Ohio
• Minimum of 2 years of experience in a healthcare or long-term care setting
• Strong communication, organizational, and problem-solving skills
• Knowledge of Medicare/Medicaid regulations and discharge planning
• Commitment to resident-centered care and ethical practice
