Position Summary
Maria Joseph Nursing and Rehabilitation is seeking an organized and compassionate Discharge Planner to coordinate safe, timely, and effective discharge plans for our residents. This role serves as a key liaison between residents, families, healthcare providers, and community resources to ensure continuity of care after discharge.
Key Responsibilities
- Coordinate and manage resident discharge planning in collaboration with the interdisciplinary care team.
- Assess resident needs and arrange appropriate post-discharge services such as home health, DME, therapy, hospice, or follow-up appointments.
- Communicate discharge plans clearly with residents and families.
- Ensure all discharge documentation is complete, accurate, and compliant with regulations.
- Work closely with physicians, nursing staff, therapy teams, and insurance providers.
- Monitor discharge outcomes and address barriers to safe transitions of care.
- Maintain compliance with state, federal, and facility regulations.
Qualifications
- Experience in skilled nursing, case management, discharge planning, or healthcare coordination preferred.
- Strong organizational, communication, and documentation skills.
- Knowledge of community resources and post-acute care services.
- Ability to manage multiple cases while maintaining attention to detail.
