Community-Based Case Manager

Marguerite’s Place empowers New Hampshire families to thrive. For over 30 years, we’ve supported women and children in Nashua through housing, community programs, and individualized supports that help families move from crisis to stability and long-term success.
We’re looking for a Community Case Manager to be a welcoming, first point of contact for families seeking support. This role bridges our community and housing programs — meeting people where they are, helping determine eligibility for transitional housing, and connecting families to the resources they need while they wait for placement or work toward stability.
The ideal candidate is compassionate, organized, and comfortable engaging with community members and partners. You’ll play a key role in helping families navigate housing, access services, and take steps toward greater self-sufficiency.
What You’ll Do/Key Responsibilities
Outreach and Case Management
Serve as the first contact for new program applicants, assessing needs, connecting families with supports, and providing short-term case management and referrals for housing, employment, childcare, and basic needs. Supporting our Phase 2 residents with case management, as well as our after-care and community-based families.
Housing Support
Help families prepare for housing, apply for permanent housing and create individualized plans to support long-term stability.
Resource Coordination
Build partnerships with local agencies and community providers to strengthen the network of support available to families.
Crisis & Advocacy Support
Respond to crisis situations with care and professionalism, ensuring families feel supported and connected to appropriate resources.
Documentation / Reporting / Data Collection
Maintain accurate documentation, track progress toward goals, and ensure timely follow-up and reporting.
Collaboration with Programs Team
Work closely with residential and community program teams to provide holistic family support and participate in evening workshops and events held several times a week.
What You Bring
- Bachelor’s or associate degree in social work, human services or related field preferred
- Professional experience in case management, housing services, or related social service work expected. Comparable lived experience value and considered.
- Understanding of (or willingness to learn) housing systems and barriers facing low-income families
- Strong communication, organization, and relationship-building skills
- Commitment to trauma-informed, strengths-based practice
- Reliable transportation and ability to work a flexible schedule with evening hours
Job Type: Full-time
Pay: From $48,000.00 per year
