Overview
This dynamic position involves working with those who have become involved in the legal system, who also have substance use disorders and/or other serious and persistent mental illnesses. The goal of the CCBHC program is to support clients in accessing community resources to assist them in making healthy choices in their lives so that they can change their pattern of legal entanglements, and have the opportunity to lead a productive and fulfilling life.

Duties

Under the direction of the Behavioral Health Court Coordinator and the Senior Clinical Case Manager, the case manager is responsible for providing clients with the tools they need through rehabilitative programming, reinforcement, referrals to community resources, and monitoring. The CCBHC program Case Manager will:

  • Develop comprehensive individualized service plans and direct clients to resources, assisting clients with applications, communication, etc as needed.
  • Maintain frequent communications with all provider agencies regarding client progress.
  • Maintain ongoing communications with the Department of Corrections, Probation, and Parole offices.
  • Report to courts on clients’ level of compliance with, or violation of, program requirements and their individualized service plan.
  • Conduct regular monitoring meetings with clients and maintain continual assessment of progress.
  • Develop and maintain individual records, in accordance with local, state and federal guidelines, including urinalysis results, and prepare reports for review by courts, attorneys, etc.
  • Attend regular Behavioral Health Court team meetings and all meetings mandated by Mental Health Court State Coordinator.
  • Assist Coordinator with updating program needs, i.e. brochure, handbook, policy and procedures manual, as needed.
  • Maintain compliance with the statewide data base, and enter all required data into case management forms.
  • Maintain appropriate client confidentiality in accordance with HIPAA, 42 CFR, pt2, and any other relevant guidelines.
  • Run appropriate psychoeducational groups as-needed.

Skills & Experience:

  • SKILLS/EXPERIENCE/TRAINING REQUIRED:

Duties require knowledge and experience working with individuals experiencing chronic and persistent mental illness, substance use disorders, and/or co-occurring disorders. Must have, at minimum, a Bachelor’s degree in a related field, and three to five years of related work experience in these fields, or an equivalent combination of education and experience.

Preference given to those who have a LADC, and/or are working toward (or have) a Master’s degree in a related field. Knowledge of the judicial and corrections systems preferred.

  • Ability to understand and appreciate the dynamics of working with persons with substance use disorders and/or other mental illnesses, persons in crisis, and persons seeking to enhance personal growth and functioning toward more effective living.
  • Ability to communicate effectively both verbally and in writing; to establish positive public relations; and to interact effectively with a wide variety of people.
  • Ability to manage a large and varied caseload.
  • Ability to work on a multi-disciplinary team.
  • Ability to work under pressure.
  • Ability to work effectively with community referral sources and caretakers.
  • Ability to maintain up-to-date documentation.
  • Ability to work under pressure.
  • Demonstrated knowledge of therapeutic interventions and treatments specific the client population.
