Job is Mon-Friday with on call weekend rotation

Overview
We are seeking a dynamic and dedicated Unit Manager specializing in Long Term Care (LTC) to lead and coordinate comprehensive patient care within our Skilled Nursing Home. This role offers an exciting opportunity to oversee daily operations, ensure high-quality nursing services, and foster a compassionate environment for residents. Duties

  • Lead and supervise nursing staff, including nurse managers and direct care providers, ensuring efficient team management and fostering a positive work environment.
  • Oversee patient care activities, including assessments, medication administration, IV infusion procedures, catheterization, and wound care, ensuring adherence to best practices and safety protocols.
  • Coordinate interdisciplinary care plans for residents with diverse needs such as dementia care, behavioral health challenges, geriatrics, and memory care.
  • Ensure compliance with healthcare regulations including HIPAA, NCQA standards, and state licensing requirements; maintain accurate medical records using EMR systems like Cerner or Athenahealth.
  • Manage clinical operations related to medical documentation, diagnostic evaluations, medical imaging coordination, and discharge planning.
  • Facilitate staff training on infection control techniques, aseptic procedures, emergency response protocols (including ventilator management), and basic life support certification.
  • Conduct regular audits of medical records and documentation to ensure accuracy in ICD coding (ICD-9/ICD-10), CPT coding for procedures like gastrostomy or spinal taps, and DRG reporting.
  • Implement quality improvement initiatives focusing on patient safety, pain management, acute pain control strategies, and utilization review processes.
  • Collaborate with physicians during primary care visits or hospital transfers involving ICU or Level I/II trauma center experiences; coordinate with external specialists for diagnostic evaluation or laboratory testing.

Skills

  • Proven experience managing nursing teams within long-term care facilities or similar healthcare settings such as assisted living or skilled nursing homes.
  • Extensive clinical knowledge in areas including geriatrics, dementia care, behavioral health, acute care nursing (medical-surgical), ICU experience, emergency medicine procedures, and hospital medicine practices.
  • Proficiency in EMR/EHR systems such as Cerner or Athenahealth; familiarity with medical imaging software is a plus.
  • Strong understanding of physiology principles related to patient assessment and vital signs monitoring; skilled in physical examinations and specimen collection/processing.
  • Expertise in medication administration protocols—including injections—and IV insertion techniques; competent in sterile processing and infection control standards.
  • Ability to interpret medical terminology accurately; experienced in documentation related to CPT coding for specialized procedures like cardiac catheterization or spinal taps.
  • Supervisory skills with demonstrated success in team management; capable of training staff on HIPAA compliance and patient confidentiality standards.
  • Knowledge of healthcare laws including workers' compensation law; adept at discharge planning and working with individuals with disabilities or developmental challenges.
  • Excellent communication skills for triage assessments, family coaching sessions, and collaborating with multidisciplinary teams across pediatric to neonatal levels of care when needed.
