Key Responsibilities
Transportation Duties
- Coordinate and assist with scheduling resident transportation for medical appointments
- Safely transport residents to and from appointments using the facility vehicle
- Assist residents during transport, including wheelchair and mobility support
- Communicate with nursing staff regarding resident needs and appointment details
- Maintain vehicle cleanliness, fuel levels, and report maintenance needs
- Provide direct resident care in accordance with care plans
- Assist residents with activities of daily living (ADLs)
- Support nursing staff with routine care tasks
- Monitor and report changes in resident condition
- Ensure resident safety, dignity, and comfort at all times
- High School Diploma or GED (required)
- Certified Nursing Assistant (CNA) certification (required)
- Valid driver’s license with a clean driving record (required)
- BLS/CPR certification (required)
- Experience in a skilled nursing facility or long-term care setting preferred
- Ability to lift, transfer, and assist residents as needed
- Strong communication, organizational, and teamwork skills
