We are seeking a dependable, compassionate, and motivated Licensed Practical Nurse (LPN) to join our team. The ideal candidate will be comfortable working with a diverse patient population and have a strong interest in primary care, behavioral health, and addiction medicine.

Position Summary

The Licensed Practical Nurse will work closely with the healthcare provider and other members of the clinical and administrative team to support safe, efficient, and high-quality patient care.

This position includes both clinical and administrative nursing responsibilities and is ideal for an LPN who enjoys outpatient nursing, patient education, organization, and building positive relationships with patients.

Responsibilities

• Obtain and accurately document patient vital signs, medical history, medication information, allergies, and other clinical information.
• Prepare patients for examinations, evaluations, and procedures.
• Assist the healthcare provider with patient visits and clinical procedures within the LPN scope of practice.
• Administer medications and injections as ordered and permitted by Pennsylvania law and organizational policy.
• Perform specimen collection, point-of-care testing, urine drug screening, and other clinical testing as assigned.
• Assist with TB testing, laboratory services, physical examinations, and other services provided by the practice.
• Support patients receiving mental health and substance use disorder treatment in a professional, respectful, and nonjudgmental manner.
• Provide patient education regarding medications, treatment plans, follow-up care, and provider instructions.
• Communicate patient concerns, changes in condition, abnormal findings, and other pertinent information to the appropriate provider.
• Maintain accurate, timely, and confidential documentation within the electronic health record.
• Assist with prescription refill requests, prior authorizations, referrals, laboratory results, and other clinical coordination as directed.
• Maintain appropriate infection-control and safety practices.
• Ensure examination rooms and clinical areas are clean, organized, stocked, and prepared for patient care.
• Maintain appropriate medication, supply, and clinical inventory.
• Assist with telephone communication and patient follow-up when clinical nursing input is required.
• Protect patient confidentiality and comply with HIPAA and all applicable federal and state regulations.
• Work collaboratively with providers, administrative staff, and other members of the healthcare team.
• Perform additional nursing duties consistent with LPN scope of practice and the needs of the organization.

Qualifications

• Current and unrestricted Pennsylvania LPN license required.
• Graduate of an accredited practical nursing program.
• Current CPR/BLS certification preferred.
• Previous experience in primary care, behavioral health, addiction medicine, outpatient care, or a related setting is preferred but not required.
• Strong communication and interpersonal skills.
• Ability to maintain professionalism and confidentiality.
• Strong organizational skills and attention to detail.
• Ability to work both independently and collaboratively as part of a healthcare team.
• Comfortable working with individuals from diverse backgrounds and with patients experiencing mental health or substance use concerns.
• Basic computer skills and ability to learn electronic health record systems.
