What are the responsibilities and job description for the Public Health Nurse position at MANITOWOC COUNTY?
Work Monday - Friday with holidays off and join Manitowoc County's team as a Public Health Nurse. This position provides population focused community-oriented nursing care based on the core functions of public health and:
- Performs surveillance, investigation, prevention and control of communicable and chronic diseases.
- Identifies population groups, families and individuals at high risk of illness, injury, disability, or premature death.
- Provides leadership for community health improvement activities.
- Addresses community health needs by collecting, reviewing and assessing data to determine service and program needs.
- Develops intervention strategies aimed at promoting healthy environmental, social, and economic conditions for all populations.
- Collaborates with public and private organizations to address community health needs.
- Evaluates effectiveness of public health programs and interventions.
Hours are Monday - Friday 8:30 AM - 4:30 PM
