POSITION SUMMARY
Responsible for providing administrative support for the medical providers assigned to the Women’s Health care team. Responsible for assisting patients with questions, appointments, referrals, travel and any other services required for care by the Women’s Health team. Provides support to the Women’s Health Case Manager in coordination and tracking of care provided to clients under the women’s health umbrella. Coordinator maintains a working relationship with other case managers, case manager supports and patient navigators to correlate and streamline care. Takes direction from the Women’s Health Case Manager and is directly supervised by the Assistant Chief Nursing Officer/Outpatient Services Manager.
PRINCIPLE DUTIES AND RESPONSIBILITIES
Women’s Health Patient Coordinator I
1. Answers in-coming calls, and schedules patient appointments for Women’s Health.
2. Carries out receptionist duties including re-scheduling and confirming patient appointments.
3. Prioritizes and communicates information to maintain an accurate appointment system.
4. Handles a high call volume quickly and accurately, while maintaining courtesy to the patients.
5. Maintains accurate schedule in the appropriate EHR.
6. Handles all scheduling queries from patients and hospital administration.
7. Prepares reports and statistics, as required by supervisor, for reporting to the hospital administration.
8. Ensures office and workspace is adequately equipped, supplied, maintained.
9. Prepares purchase and work orders as needed.
10. Copies information and distributes to Outpatient Program staff.
11. Maintains up-to-date telephone list of Maniilaq Health Center Programs.
12. Coordinates schedules between providers, nurses, staff and other Programs.
13. Maintains strict confidentially of all patient and Outpatient business within the program.
14. Engages in professional development activities to keep abreast of current practice in women’s health.
15. Assists with the tracking and follow up of care achievements including abnormal findings in collaboration with case management.
16. Confirms Women’s Health patients are attending necessary appointments both at MHC and outside facilities.
17. Tracks referrals to outside support programs such as WIC and ELF.
18. Performs clerical duties as required to support the women’s health care team i.e. letters, faxing and copying.
19. Maintains files in a secure and organized manner
20. Notifies patients of normal labs when directed by the provider or case manager
21. Assists with the referral process, including patient travel, in collaboration with provider/case manager.
22. Participates in departmental and organizational performance improvement activities.
23. Treats all patients and staff with dignity and respect.
24. Proficient in use of the current EHR.
25. Collaborates with appropriate case manager concerning problems in referring patients.
26. Assists patients with referrals to appropriate agencies for financial, nutritional, and other support services as appropriate.
27. Other duties as assigned.
Women’s Health Patient Coordinator II
1. All of the above duties and responsibilities for Women’s Health Patient Coordinator I.
2. Completes an approved breastfeeding support/coach course. Must be a minimum of 15 hours.
3. Provides efficient support/education to all prenatal patients on breastfeeding.
4. Offers support to breastfeeding or potentially breastfeeding mothers directly after birth and throughout the first year of the infant’s life.
MINIMUM REQUIREMENTS
Women’s Patient Coordinator I
High school diploma or GED required. Ability to utilize various computer programs and type well enough to carry out job duties efficiently. Exceptional communication and people skills required. Must be able to multitask and handle multiple providers scheduling and patient care needs. Health industry experience required. Medical terminology course is required within 1 year. Requires an excellent work ethic, critical thinking and problem solving skills. Previous experience in women’s health preferred.
Women’s Patient Coordinator II
Meet the requirements of Women’s Patient Coordinator I, plus completion of an approved breastfeeding course with a minimum of 15 learning hours.
