Manifest Behavioral Health is a growing behavioral health organization providing Detox, Mental Health, PHP, and Intensive Outpatient (IOP) services to adults struggling with mental health disorders, substance use disorders, and co-occurring conditions.
Our mission is to provide compassionate, evidence-based treatment while helping clients build sustainable lives in recovery. We are seeking a highly organized, client-focused Case Manager to support our growing IOP and PHP programs.
This position is ideal for someone who enjoys working directly with clients, coordinating care, solving problems, and helping individuals overcome barriers to treatment and long-term recovery.
Position Summary
The Case Manager is responsible for managing an assigned caseload of clients receiving behavioral health and substance use treatment services. The Case Manager serves as a liaison between clients, therapists, medical providers, family members, outside agencies, and insurance providers to ensure continuity of care and successful treatment outcomes.
This role requires strong communication skills, documentation experience, organizational abilities, and a genuine passion for helping individuals improve their lives.
Essential Duties & ResponsibilitiesClient Case Management
- Maintain an active caseload of clients enrolled in PHP and IOP services.
- Complete biopsychosocial assessments and case management assessments as assigned.
- Assist clients with obtaining community resources, housing referrals, employment resources, transportation, insurance benefits, and social services.
- Coordinate medical, psychiatric, therapy, and outside provider appointments.
- Monitor client progress toward treatment goals.
- Participate in multidisciplinary treatment team meetings.
Treatment Coordination
- Collaborate with therapists, counselors, medical providers, and leadership regarding client progress.
- Assist with treatment plan updates and discharge planning.
- Support continuity of care and referral coordination.
- Maintain communication with family members and support systems when appropriate and authorized.
Documentation & Compliance
- Complete case management notes in the EMR within required timelines.
- Document client contacts, referrals, discharge planning, and resource coordination.
- Assist with ASAM and LOCUS information gathering as needed.
- Ensure documentation meets payer, accreditation, and regulatory requirements.
- Participate in quality assurance and chart audit activities.
Discharge Planning
- Develop comprehensive discharge and aftercare plans.
- Coordinate outpatient referrals, support groups, recovery resources, psychiatric follow-up, and community services.
- Assist clients with transition planning to lower levels of care.
Crisis Support
- Recognize and report safety concerns, relapse risks, and behavioral health emergencies.
- Participate in crisis intervention and client stabilization efforts when needed.
- Communicate urgent concerns to clinical leadership immediately.
QualificationsRequired
- High School Diploma, Associate's Degree, or Bachelor's Degree in Psychology, Social Work, Human Services, Behavioral Health, or related field.
- Minimum one year of experience in behavioral health, substance use treatment, mental health services, social services, or case management.
- Strong written and verbal communication skills.
- Proficiency with electronic documentation systems.
- Valid California Driver's License and reliable transportation.
Preferred Certifications
- CADC-I or CADC-II
- CATC I-V
- SUDCC I-IV
- RADT (Registered Alcohol Drug Technician)
- CCAPP certification pathway candidates
- CPR and First Aid Certification
Preferred Experience
- Mental Health IOP or PHP experience
- Substance Use Disorder treatment experience
- Detox or Residential treatment experience
- ASAM familiarity
- LOCUS familiarity
- KIPU or behavioral health EMR experience
- Insurance authorization and utilization review support experience
