Manhattan Gastroenterology is seeking a highly organized, detail-oriented, and patient-focused AfterCare Coordinator to join our team. The AfterCare Coordinator plays an important role in ensuring that patients receive timely follow-up care and that clinical information, test results, referrals, and outstanding orders are accurately managed and documented.
The ideal candidate will have experience working in a private medical practice, gastroenterology office, ambulatory surgical center, or similar healthcare setting and be comfortable managing a high volume of patient follow-up while prioritizing tasks and meeting deadlines.
The AfterCare Coordinator will work closely with physicians, clinical staff, and the Quality Assurance & Operations Manager to ensure that patient needs are addressed efficiently and that all follow-up responsibilities are completed accurately and within established timeframes.
Key Responsibilities
Review and manage assigned AfterCare tasks and physician orders in the EMR.
Obtain, review, and attach clinical documents, laboratory results, imaging reports, procedure reports, and other relevant records to the appropriate patient chart.
Review laboratory and diagnostic results and ensure they are routed appropriately for physician review and follow-up.
Assist with coordinating patient referrals, follow-up appointments, procedures, imaging, laboratory testing, and other recommended care.
Follow up on outstanding orders and referrals to ensure patients do not fall through the cracks.
Contact patients regarding physician recommendations, test results, follow-up appointments, referrals, and other non-emergency clinical follow-up needs, in accordance with practice protocols.
Manage a high volume of inbound and outbound patient calls while maintaining professionalism and providing excellent customer service.
Schedule and confirm patient appointments and assist patients with navigating the next steps in their care.
Identify patient needs, clarify concerns, research issues, and provide appropriate solutions or escalate concerns to the appropriate clinical or administrative resource.
Respond to physician and clinical staff tasks in a timely and accurate manner.
Maintain accurate documentation of patient communications, follow-up attempts, and completed tasks within the EMR.
Ensure all patient interactions and follow-up activities are documented according to practice standards.
Follow established communication scripts, workflows, policies, and guidelines when communicating with patients.
Recognize situations that require escalation to a nurse, physician, supervisor, or other appropriate resource.
Protect patient confidentiality and comply with HIPAA and all applicable healthcare regulations and practice policies.
Work collaboratively with the AfterCare team, physicians, nurses, medical assistants, front desk, scheduling, and other departments to ensure continuity of patient care.
Assist with identifying and addressing outstanding AfterCare work, including pending labs, procedures, imaging, referrals, and other follow-up items.
Maintain organization and productivity while managing multiple priorities and a high-volume workload.
Perform other duties and responsibilities as assigned.
Qualifications
1–2 years of experience in a medical office, private practice, ambulatory surgical center, or similar healthcare environment preferred.
Previous experience in patient aftercare, referral coordination, medical scheduling, care coordination, or medical office administration strongly preferred.
Experience working with an Electronic Medical Record (EMR/EHR) system.
Strong computer and data-entry skills.
Excellent verbal and written communication skills.
Strong organizational skills and attention to detail.
Ability to prioritize and manage multiple tasks in a fast-paced environment.
Comfortable handling a high volume of patient calls and follow-up responsibilities.
Ability to communicate effectively and professionally with patients, physicians, nurses, and administrative staff.
Ability to work independently while also functioning effectively as part of a team.
Strong problem-solving and follow-through skills.
Ability to maintain patient confidentiality and handle sensitive medical information appropriately.
Bilingual skills are a plus.
