**WE URGENTLY ARE LOOKING TO HIRE MONDAY-FRIDAY 9am-3:30pm FOR OUR CCS PROGRAM**


Title: Direct Support Professional; Reports Directly To: Program Manager

Starting Wage: $16.00/hour (BASE WAGE), Increases Depending On Experience; Differentials of up to $1.00 per hour for weekend and graveyard shifts,

Evaluations provided and annually. Merit raises available up to 5% annually.


DESCRIPTION

The direct support professional assists individuals with wide ranging disabilities in achieving their goals for living, learning, and working in the community. Direct Support Professionals are responsible for supporting individuals in a variety of environments including their home, place of employment, and in the community. Direct Support Professionals follow person centered planning and are there to assist with advocacy, skill building, ADL, IADL and building of social capital in accordance with their Individual

Service Plans.


GENERAL RESPONSIBILITIES

● Develop respectful relationships with persons served, valuing their choice, preferences, abilities and opinions;

● Be alongside the person in all areas of life at work, home or in the community;

● Support persons served to speak for themselves and engage in full community participation;

● Learning about the individuals wants, needs, goals, care plan and advocating for their independent choice making;

● Educate community members and employers about supported employment, customized employment, and disability awareness and rights;

● Facilitate communication among the client, community employer, service coordinator, interdisciplinary team, and co-workers;

● Teaching your favorite hobby or encouraging them to express creativity in the form of crafts & special interests;

● Attending medical appointments and advocating with the person for appropriate care;

● Helping the person plan time to hang out with friends, socialize at gatherings, etc.;

● Providing physical support during Activities of Daily Living (meal time, teeth brushing, restroom, hygiene routines, etc.) gardening & farm/barn activities;

● Teaching valuable life skills: such as laundry, shopping and cooking, medication management;

● Provide transportation (agency vehicle) to and from medical appointments, fun activities, or work;

● Assist persons served with attending medical appointments, and advocate for appropriate treatment when needed;

● Assist persons served in integrating into the community and engaging in activities that the individual enjoys;

● Provide guidance and behavioral support, implementing behavioral support plans/interventions as described for each individual;

● Train clients on quality task completion, clocking in/out, following workplace rules, maintaining professionalism and boundaries, communication with coworkers and supervisors or other areas of growth as needed;

● Monitor and maintain an updated Job One Sheet to ensure continuity of support;

● Follow workplace codes of conduct at the assigned job site;

● Analyze how to maximize naturally occurring supports, building on the organization’s training and development programs to ensure that the client will have adequate on-going support;

● Maintain close contact with both the employee, employer and Employment Supervisor to resolve problems and continue to evaluate performance, work behavior, and production;

● Support problem-resolution processes by coaching the employer and the employee, while respecting the business culture, organizational processes, and policies and procedures;

● Facilitate communication among the client, community employer, service coordinator, interdisciplinary team, and co-workers;

● Stay abreast of current best practices, supported employment trends, and legislation;

● Have knowledge and understanding of all Individual Specific Plans, including the ISP, therapy support plans, and healthcare plans;

● Ensure thorough and timely reporting, including daily progress notes, incident reports and clock ins;

● Maintain a professional appearance and attitude at all times;

● Complete all required trainings as directed by the personnel policies, your direct supervisor, and the training coordinator;

● Attend client team meetings to report on progress;

● Maintain a professional boundary with clients; do not disclose personal information;

● Maintain regular and prompt attendance;

● Understand and follow principles of incident management;

● Must be able to physically support individuals with physical limitations to include gait belt assistance, transfer in and out of wheelchairs, partial and full physical assistance may be required

● Must be able to lift twenty-five (25) pounds above the shoulders, fifty (50) pounds repeatedly, and support seventy to one hundred (70-100) pounds occasionally.

Pay: $16.00 - $17.00 per hour
