As a Case Manager, you'll play a vital role in supporting clients as they navigate their mental health journey. Through collaboration, advocacy, and individualized care planning, you'll help connect members with the resources and services they need to achieve greater stability and live as independently as possible within their communities.
Roles & Responsibilities
The Case Manager (also known as a Community Integration Worker) is responsible for identifying, assessing, planning, coordinating, monitoring, and evaluating the services and supports needed by members whose mental health diagnosis impacts their ability to maintain stability.
Working collaboratively with each member, the Case Manager develops individualized goals and provides ongoing support to help manage mental health symptoms while connecting members to community resources. Areas of support may include emotional, psychological, social, financial, medical, dental, educational, vocational, recreational, and housing needs.
Necessary Skills
- Strong time management, organizational, and multitasking abilities.
- Ability to prioritize tasks, solve problems, and perform effectively in a fast-paced environment.
- Demonstrated ability to meet deadlines and maintain accurate, timely documentation.
- Excellent verbal and written communication skills.
- Reliable transportation and willingness to transport clients using a personal vehicle.
- Ability to develop individualized 90-day treatment plans with measurable goals.
- Complete daily case notes and submit documentation on time.
- Basic computer proficiency.
Pay
- Up to $25/hr with potential for differential pay rates if desired.
Minimum Requirements
- Must have Active MHRT-C, or MHRT-C provisional.
- Valid Maine Driver’s License.
- Current vehicle inspection, registration and auto insurance.
Preferable Requirements
- At least one year of experience in the mental health field.
- Experience using KEPRO.
- Familiarity with electronic health documentation systems.
- Knowledge of qualifying clinical letters.
- Understanding of MaineCare terminology and services.
