- Develop, implement, and monitor quality improvement initiatives aligned with ACO and CMS requirements.
- Oversee accurate and timely submission of quality reporting data to meet regulatory and contractual obligations.
- Analyze performance metrics and identify opportunities for improvement in clinical outcomes and cost efficiency.
- Collaborate with providers, clinical teams, and administrative staff to ensure compliance with ACO standards.
- Lead care coordination strategies to reduce unnecessary utilization and improve patient experience.
- Serve as the subject matter expert for value-based care programs, including education and training for providers and staff which will include coding, documentation and EMR workflows.
- Maintain knowledge of federal and state regulations impacting ACO participation and quality programs.
- Prepare and present performance reports to senior leadership, highlighting trends, risks, and opportunities.
- Partner with IT and data analytics teams to optimize reporting tools and workflows.
- Ensure adherence to HIPAA and organizational compliance standards in all quality-related activities.
- Participate in strategic planning for population health initiatives and quality incentive programs.
- Partners with OHVN and organizations within OHVN/ACO to achieve goals and review data analytics.
- Other duties as assigned to support organizational goals for value-based care and quality improvement.
- Strong knowledge of CMS quality programs, ACO requirements, and value-based care models.
- Minimum 1-3 years of experience in healthcare quality improvement, compliance, or performance management preferred.

- Excellent analytical, organizational, and communication skills.
- Ability to lead cross-functional teams and manage complex projects.
Preferred:
- Master’s degree in healthcare administration or related field.
- Certifications such as CPHQ (Certified Professional in Healthcare Quality) or Lean Six Sigma Black Belt.
- Experience with electronic health records (EHR) and data reporting systems.
- Prior experience in a leadership role within a quality or population health department.
