Support Clients Through One of the Most Important Stages of Recovery
Talbot Health Services is seeking an Inpatient Case Manager to join our residential treatment team. This position works directly with individuals receiving residential substance use treatment services and helps ensure clients leave treatment with a clear plan, strong support system, and access to the resources necessary for continued success in recovery.
From admission through discharge, the Inpatient Case Manager serves as a key member of the treatment team by helping clients address barriers, connect with community resources, and prepare for life after treatment.
Essential Responsibilities
- Provide case management services to individuals participating in residential treatment.
- Coordinate discharge planning and aftercare services.
- Assist clients with housing placement, employment resources, transportation, healthcare access, identification documents, public benefits, and recovery support services.
- Coordinate referrals to outpatient treatment providers, recovery housing programs, medical providers, and community agencies.
- Collaborate with counselors, nursing staff, medical providers, residential staff, and referral sources.
- Monitor progress toward treatment and discharge goals.
- Participate in multidisciplinary treatment team meetings.
- Maintain accurate and timely documentation.
- Ensure compliance with HIPAA, 42 CFR Part 2, CARF standards, Medicaid requirements, and agency policies.
Qualifications
Required
- Active Ohio CDCA certification.
- High school diploma or equivalent.
- Valid driver's license and reliable transportation.
- Strong communication and organizational skills.
Preferred
- Experience working in residential treatment, behavioral health, addiction treatment, healthcare, corrections, or social services.
- Experience with discharge planning and community resource coordination.
