Job Description: Are you passionate about helping individuals and families overcome barriers, connect with resources, and access the support they need to thrive? The Macon County Health Department is seeking a compassionate, organized, and community-focused Better Birth Outcomes Comprehensive Navigator to join our Maternal and Child Health team.
In this role, you’ll work directly with individuals and families enrolled in the Better Birth Outcomes Comprehensive (BBO-C) Program, providing assessment, education, referrals, follow-up, and assistance navigating healthcare and community resources. You’ll build meaningful relationships with clients while helping address barriers that may impact healthy pregnancies, healthy families, and positive birth outcomes.
What You’ll Do
- Assess client eligibility for the BBO-C Program using established income guidelines and the BBO-C Vulnerability Index.
- Provide individualized assessments, referrals, follow-up, and case management in collaboration with WIC and other services.
- Partner with families to identify barriers and connect them with appropriate resources and support.
- Help clients understand and access Managed Care Organization (MCO) benefits, care coordination, and other available services.
- Educate clients on the importance of establishing and maintaining a medical home.
- Complete social determinants of health screenings and identify opportunities for additional support.
- Provide health and diagnosis-specific education according to BBO-C program requirements.
- Conduct in-person client visits, including home visits when requested by the client.
- Conduct community outreach at FQHCs, pregnancy centers, prenatal offices, and other community locations.
- Deliver required program curriculum and educational materials.
- Complete accurate and timely documentation using the Illinois Department of Health and Human Services data-entry system.
- Collaborate with families, healthcare providers, schools, community organizations, and other professionals to coordinate services.
- Assist with the development of informational and outreach materials.
- Support training and job shadowing for new employees and interns.
- Maintain knowledge of MCHD programs to help clients connect with additional services when appropriate.
Qualifications Required
- Associate Degree in behavioral science, social science, a health-related field, or a related area
- Strong interpersonal and communication skills
- Ability to establish and maintain positive relationships with clients, community members, and professional partners
- Ability to effectively work with individuals and families from diverse backgrounds
- Computer proficiency with the ability to organize, document, and maintain information accurately
- Dependable vehicle available for use during work hours
- Valid Illinois Driver’s License and appropriate automobile insurance
- Ability to travel throughout the community and enter and exit a vehicle multiple times throughout the workday and in varied weather conditions
- Ability to adapt professionally and respectfully to a variety of client home and living environments
Preferred
- Bachelor’s Degree in behavioral science, social science, a health-related field, or related area
- Previous experience working in child, family, or community services
- Experience with case management, care coordination, community outreach, or resource navigation
- Experience working with maternal and child health, prenatal services, WIC, public health, or community-based programs
