Job Summary
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our healthcare team, dedicated to providing comprehensive primary care from age 18 up. The ideal candidate will have a minimum of 5 years in family practice/IM and already have a large patient panel so hopefully some of your patients will follow you to a DPC office. In this role, you will deliver high-quality medical services, perform thorough patient assessments, and develop personalized treatment plans. Your energetic approach will help foster a positive patient experience while ensuring adherence to clinical standards and best practices. This position offers an exciting opportunity to make a meaningful impact on community health through proactive care, education, and collaboration with multidisciplinary teams. This is a Direct Primary Care office, independently owned by a nurse practitioner. Our patients pay a monthly membership to establish with our office and we do not bill any insurance companies. We draw labs in the office and send them to Quest and offer patients a very reduced rate on labs. We work with imaging centers to get the lowest costs on any diagnostic tests that are needed. We intentionally have smaller patient panels so that our patients can be seen in a timely manner and our patients have longer appointment times so that we can really get to know our patients.
Responsibilities
- Conduct detailed physical examinations, health assessments, and vital signs measurements for patients.
- Diagnose acute and chronic illnesses, utilizing diagnostic evaluation tools such as laboratory tests, imaging studies, and clinical judgment.
- Develop and implement individualized treatment plans, including prescribing medications, administering injections, and managing follow-up care.
- Perform procedures such as minor wound care, skin biopsies, draw labs in the office
- Collaborate with specialists in areas like gastroenterology, cardiology, infectious disease care, and behavioral health to coordinate comprehensive patient management.
- Utilize electronic health record (EHR) for accurate documentation of patient encounters, medical records, CPT coding, ICD-10 coding, and compliance with HIPAA regulations.
- Participate in telehealth consultations, case management activities, discharge planning, and post-acute care follow-up to ensure continuity of care across settings.
Requirements
- Valid Family Nurse Practitioner license or certification recognized by relevant licensing boards.
- Proven experience in primary care settings with proficiency in medical office operations and EMR/EHR.
- Strong clinical skills including patient assessment, physical examinations, diagnostic evaluation, and medication administration.
- Hands-on experience with procedures such as phlebotomy, sterile processing techniques, and basic life support (BLS)
