Qualifications:
- CPR Certification (Preferred)
- Must have Restorative experience
- Certified Nursing Assistant (CNA) license required
- 1 year of CNA experience required
- Successful completion of a Restorative Aide program and examination
- High School diploma or GED equivalent
- Must be 18 years or older

- Coordinate with nursing, therapy, and administrative staff to plan and execute restorative care
- Develop and maintain restorative care documentation, including resident files and progress notes
- Review physician orders and therapy requests
- Evaluate and select appropriate therapy activities based on resident history
- Assist in developing and updating MDS and care plans
- Educate and guide residents during restorative care activities
- Encourage resident participation in ADLs and mobility exercises
- Maintain safety standards and report care modifications as needed
- Perform other related duties as assigned
- Proficiency in English (reading, writing, speaking)
- Ability to make independent decisions and work collaboratively
- Skilled in using sensory aids or prosthetics when necessary
- Capable of lifting at least 25 pounds and performing physical tasks
- Must meet facility health guidelines, including physical exams
