- You Will Play a Critical Role: As an LPN /RN MDS Coordinator, you'll conduct and coordinate resident assessments, ensuring compliance with federal, state, and local standards.
- Team Collaboration: Work closely with the Interdisciplinary Care Plan Team to develop comprehensive care plans for each resident.
- Resident Assessment Development: Conduct and coordinate Resident Assessment Instruments (RAIs) in line with regulations and guidelines.
- Supportive Collaboration: Collaborate with the Care Plan Team to craft personalized resident care plans.
- Evaluation and Reporting: Assess resident conditions, ensuring special assessments are conducted when necessary, and provide necessary data for the Facility's Quality Assurance Program.
- Active LPN/RN MO License: Current and in good standing.
- MDS Coordinator Experience: Prior experience in a similar role.
- Regulatory Knowledge: Familiarity with Medicare/Medicaid regulations and benefit guidelines.
