Responsibilities:
- Collaborate with the Admissions Director to obtain and assess referral information, including complete clinical and financial details.
- Coordinate with the Business Office to ensure accurate and complete financial verification prior to admission.
- Conduct facility tours and engage families to ensure commitment and satisfaction.
- Manage the admission process, explaining policies, ensuring all paperwork is complete, and supporting a smooth transition.
- Track and communicate status of pending referrals and upcoming admissions to appropriate staff.
- Notify the Marketing Director of any referral source concerns or issues.
- Provide exceptional customer service and help orient new residents to the facility.
- Support floor staff with resident needs as schedule allows.
- 2–3 years of experience in a SNF, subacute, rehab, or healthcare facility required.
- Active LPN / RN license required.
- Excellent organizational and documentation skills.
- Strong telephone and communication skills with the ability to handle referrals and family inquiries.
- Knowledge of the managed care industry is a plus.
- Maintain a professional image through appearance, speech, and conduct.
- Resolve concerns in accordance with company standards.
- Ensure protection of all resident rights.
